Derived from the EDI-3, the EDI-3 Referral Form assesses eating disorder risk based on dieting concerns, body weight, weight history, height, and behavioral symptoms indicative of eating disorders.

  • Can be administered in 5 minutes and includes behavioral symptom questions to help identify individuals with potential eating disorders or pathology.
  • Referral indexes are used to identify individuals who have or are at risk for eating disorders. These indexes are based on the individual’s body mass index (BMI) only; on BMI plus responses to EDI-3 questions about excessive eating concerns; and on responses to behavioral questions pertaining to eating disorder pathology.
  • Can be administered in group or individual settings by mental health professionals, school counselors, coaches, camp counselors and others.
